Long-Term Care Services

Medicaid Long-Term Care Services can provide help with your care needs in the setting of your choice. Options include care in your home, Adult Foster Homes, Residential Care Facilities, Assisted Living Facilities, and Nursing Homes. Medicaid is available to older adults (age 65 and over) and adults with physical disabilities (age 18 and over) who need help with tasks of everyday living.

Adult Foster Care

Adult foster care is an excellent choice for someone looking for an affordable, homelike setting. Informal adult foster care has been a part of our society for years. People that were unable to maintain their health while living alone moved in with family, friends, and neighbors. In an adult foster home, medical and personal care are provided in a manner that encourages maximum independence and enhances the quality of life.

Case Management Services

Case Managers can help older adults and adults with disabilities who need personal care assistance to access the services and resources they need. Services that may be available under this benefit include:

  • A visit to your home to assess your needs and develop a care plan.
  • Referral for other community-based services as needed.
  • Arrangement for you to receive supportive services such as home-delivered meals or transportation to medical appointments.
  • Regular contact with you to ensure that the services you are receiving continue to meet your needs.

 

Oregon Project Independence (OPI)

The OPI program for adults over the age of 60 currently has a waitlist across Lane County.

OPI provides limited in-home services to people who need a little help to continue living independently in their own homes. The program is available to adults age 60 and older. In addition, Senior & Disability Services is participating in a pilot program which expands these services to adults with disabilities age 19-59. The OPI program and the pilot expansion are funded by the Oregon State Legislature.

Services are provided on a sliding fee scale based on gross monthly income minus out-of-pocket medical expenses. There are no resource or income limits. Individuals are not eligible if they have Medicaid medical coverage.

Services offered through OPI may include personal care, Meals on Wheels, case management, and assistive technology such as emergency response devices. Hours for personal care are usually limited to a maximum of four or five hours per week.

 

Public Disclosure File

Senior & Disability Services maintains a public disclosure file of complaints involving residents in licensed facilities. The disclosure file also contains information about a facility's license history. Please contact our office at 541-682-4038 if you are interested in viewing the public file.

 

ADRC Aging and Disability Resource Connection of Oregon Logo

The Aging and Disability Resource Connection, or ADRC, is a free service that offers the public a single source of information and assistance on issues affecting older people, people with disabilities, and their caregivers, regardless of their income. ADRC is dedicated to:

  • Helping individuals to understand the various long-term care options available to them.
  • Enabling individuals to make informed, cost-effective decisions about long-term care.
